Significant Incident Report

The report below is to be completed by each official and submitted within 48 hours of the incident.

NOTE – New beginning in 2022-23 – from OABO, if the incident involves an OBA team, please also complete the OABO/OBA Incident form (in addition to the Significant Incident Report below). Per OBAO, “The goal is have all boards use this form to allow us to better track clubs/teams that are consistently violating the fair play code of conduct.”
